Individualized Home Supports
One-on-one, in-home assistance with daily living skills, household management, and personal goals — delivered according to your individual support plan.
Community Living Supports
Support to get out and take part in community life — medical appointments, errands, social activities, and building relationships on your terms.
Independent Living Skills Training
Hands-on coaching in the skills that support long-term independence — budgeting, meal preparation, medication reminders, and personal safety.
Respite Care
Short-term, trained relief for family caregivers, so the people who care for you get the rest they need too.
Night Supervision
Overnight support and safety monitoring for individuals who need assistance or supervision during nighttime hours.
Family & Caregiver Training
Training and coordination with family members and natural supports, so everyone involved in your care is working from the same plan.
Eligibility & funding
Our services are available to eligible individuals through Minnesota Medical Assistance home and community-based waivers. Your county case manager can help confirm your eligibility and authorize services — and we're glad to help you connect with them.
